It's an idea I've had for a while, and I actually typed it out a year or two ago.
The fanfic is a Ranma and Pokemon crossover. The timeline it takes place in is when Ranma is six-years-old and on the training trip with Genma. It also takes place after the Neko-ken training and Ranma and Genma are in the woods. Ranma decides to explore the forest a little and ends up getting lost. As he's running around and trying to find his way back, he bumps into a woman who's stepping out of an odd portal. Because he was jumping, both fall back into it.
When Ranma takes a look at the one he bumped into, it turns out the woman had a few characteristics of a Pikachu (Ears, Tail, red cheek circles), and that Ranma was now in a different world where the inhabitants had features of Pokemon. The Pikachu woman, Terra, also accidentally fell onto the small device -- that she snuck away from her father -- that allowed her to open the portal. This meant that Ranma couldn't get back to his own world for a little while, and Terra takes Ranma to meet her father -- and to face up for what she did.
Until they can get another device ready, Ranma is stuck in that place, and Terra decides that the young boy should live with her and her daughter -- who happens to have Pichu characteristics -- until he can get back to his own place. In the meantime, Ranma's still able to train in his martial arts, and makes a rival out of another kid -- with Tyrogue characteristics -- who loved to bully Terra's daughter because she still couldn't get her electricity to work right. Time also moves faster in that world, and Ranma's body aging is in sync with his own world's time. This means that when he gets back, not too much time would pass.
When he's finally able to get back a few months later, Terra gives him a small necklace -- called the Raltz Stone -- that would allow him to get in contact with her sometimes. When Ranma gets back into his own world, only a few hours had past, and so he could get back to where Genma was -- who happened to still be sleeping.
Later on when they both get to Jusenkyou, Genma gets in a lucky shot that ends up shattering the Raltz Stone and sends Ranma into the Spring of Drowned Girl. The residue from the stone, and the water in the spring react on Ranma. When he gets out of the spring, he finds that his body looks to be back around six-years-old, he's now a she, and that 'she' looks like a humanoid Raltz. When using hot water to change back, Ranma only manages to turn into a boy humanoid Raltz, but when thinking about his human form, he changes into it. It only lasts a few seconds before changing back though.
Angry over this, Ranma ended up pounding his father into the ground and accidentally reads his mind. While reading, he learned of his mother and where she lives. Ranma then proceeds to try and get to Japan on his own, and when he finally gets there, he manages to shape-shift into his human form and gets in contact with Nodoka. His mother is all excited that Ranma is there, but once she learns the truth about what happened, she talks about the Seppuku contract. Ranma, who reverted back to Raltz form, asks if she really meant it. At a nod from the woman, Ranma looks at her fearfully, shouts a few things at her, and runs out distraught. While running out, he/she accidentally taps into some of his/her Ralts form and teleports to an area where Ranma has no clue about.
That's as much as I can think about right now. I've been thinking about getting back to working on this for a while. The interesting thing is that Ranma would still be able to get in contact with Terra and the others, and later his/her form would even be able to evolve. Humanoid Raltz form looks like a six-year-old, Kirlia form looks 10, and the last form - Which looks 16 -- branches depending on gender. Gardevoir for female form and Gallade for Male.
